Nursing homes remain tax exempt under GST.
Medical shops are to be registered under GST, if turnover exceeds 20 lakhs, or to migrate to GST if already tax payer under VAT

As she is providing consultancy services to insurance company, registration under GST becomes mandatory, provided her aggregate turnover exceeds 20 lakhs. Though her clinical practice is exempt under health-care services (including medicines supplied), but the mediclaim consultancy to insurance companies would be chargeable service.
