Gst registration application rejected

my gst registration application rejected on ground that gst pct 02 and gst pct 05 not filled. I want to file all necessary form return and compliance my self and do not wish to appoint gst practioner. I mentioned in response to notice that these two are not applicable because I will file necessary return request my self and do not wish to appoint gstp. help me to resolve issue
