Dear Sir, I have paid three month salary in lieu of notice period. But my employer did not included the same in the form 16. Please advise what to do?

I am a doctor and planning to start a hospital with 2 other people (Nonmedico). I own 25% share in setup. As I will also work as a doctor am I eligible to take my consultation charges and operation charges as professional fee from hospital? Or it will be counted as salary and I cant claim upto 50% (194J). There is lot of confusion regarding the same
