You have submitted tran-1 form but have not filed it. error!

Hi, there!

In two of our cases I'm getting this error:

"You have SUBMITTED TRAN-1 form but have not FILED it. You are requested to FILE the same before submitting this return"

The problem is I'm not getting OTP when trying to file TRAN 1, I have submitted it but I'm not getting OTP.  So when I attempted to file GSTR 3B, I'm getting the above message. 

While trying to file TRAN 1, I'm getting a "SYSTEM FAILURE for GSTIN ................." 

Now I'm unable to file GSTR 3B. I'm getting OTP while trying to file GSTR 3B but when I put that in, I get the above error message saying that I need to first file TRAN 1.

I did not get any error message before or while submitting TRAN 1.  

Crux of the message:

Regarding TRAN 1 filing: I'm not getting OTP and I'm getting SYSTEM FAILURE for GSTIN...

Regarding GSTR 3B filing: I'm getting OTP but when tring to file I'm getting ..."You have submitted TRAN 1 form but have not filed it error message" and I'm unable to file GSTR 3B. 

Please help me here, as I think I may incur late fee if I file it later for no mistake of mine. I'm attaching the screenshots for your reference.
