Which itr will applicable

Sir, I am a private company employee. My monthly salary is Rs. 11,000 /- p.m. I want to show my income and want to make a IT file. But I have no payslip or bank payroll account i.e I can't prove my salary, so Can I declare my income in ITR = 1 (Sahaj) in Income from other sources head. If not then pls guide me which form is applicable for me. I have an investment in PPF also and a LIC.
