Which itr to be filled for commission agent

Dear All,

Which itr to be filed for clothing commission agent and he also draws salary as well. 

He has having two types of income 

1 commission 

2. salary

So which form to be filled for income tax return. 

Any other thing need to be shown in itr form or  not. (which is the most important things to be shown)
