Which ITR should be file for TDS deducted under the heads 194JB and 194S?

income for TDS 192 will be under Salary head. While that for sec. 194S will be declared under Schedule VDA.

So if income is earned both under this head. Which ITR should be filed?

If no income over which 194Jb is deducted, then you can go for ITR 2.
