1)I have salary income of 4.5 Lac and F&O/equity loss of 3 lac in last year..which form should I fill for ITR
2)should I get it audited from CA

You should fill itr 3. As in case of business income , itr 3 is required. And in your case, there is no requirement for tax audit from CA.

Yes, it is mandatory to show all income and losses... Otherwise it may lead to issue of scn.
