form 109 is to be mutually signed by both, i.e by u and ur boss..
and for the same city, i m not sure is it valid or not, bt one of my frnd said. if ur new office resides 50 kms away from ur old offic,, u can have such transfer.... in short, ur new offic shd not b widin range of 50 kms of old offic...
