I am a tax payer who has income from salary, income from profession and other sources (interest earned)
I would like to know which ITR form i need to use and what are the last day to pay my tax (at bank) and the last date of e-filing

you need to file itr-4 and if your income from profession is subject to tax audit, then due date of filing of return and payment of taxes is 30th sept otherwise due date is 31st july.
