As far as your reply is concern , First of all , i want to say that DON'T WORRY , & PLEASE TRY TO FOLLOW CAREFULLY , WHAT I AM SAYING NOW, OK...
1) First of all, you have to collect E-file Ack. Number for the a.y 2012-2013.( i.e, 15 digit E-file Ack no.(example) --> 496321566632178 ) and that's why you have to collect the ITR-V FOR A.Y 2012-2013 or you can note down the ACK NO. FOR A.Y 2012-2013 from the ITR-V a.y 2012-2013 ( because E-file Ack no. provided in the middle portion of the ITR-V with in a square box ) on a paper.
2) Then go to --> https://www.incometaxindia.gov.in
3) Then in forgot password option , there are 3 options & PLEASE CHOOSE the option " E-FILE ACKNOWLEDGEMNT NUMBER and YOUR BANK ACCOUNT NUMBER ".
(i.e, Here in the above BANK ACCOUNT NUMBER means --> the full digit account number you already provided/mentioned in the Income Tax return for the A.Y 2012-2013 ).
4) So, without wasting any time , choose the option " E-FILE ACKNOWLEDGEMNT NUMBER and YOUR BANK ACCOUNT NUMBER " and provide the Eifile Ack no. and Full digit bank account no.in the given box.
5) Then click on the VALIDATE button.
6) Then you MUST PROVIDE YOUR E-MAIL ID withIN THE box provided .ok..
7) Click ON SUBMIT BUTTON.
8) THEN, IT DEPT MUST PROVIDE A DUPLICATE PASSWORD IN YOUR MAIL ID ( given in Point no. 6 ) with in few seconds .
9) Then first check your mail box & collect the reset password.
10) Then once again go to --> https://www.incometaxindia.gov.in
11) provide your User ID ( i.e, Your PAN NUMBER ) & PASSWORD ( i.e, a duplicate password provided by IT DEPARTMENT IN YOUR MAIL ID mentioned in point no 8. )
12)provide your DATE OF BIRTH & CLICK ON SUBMIT BUTTON .
13) AFTER THAT , YOU MUST BE ABLE TO OPEN YOUR ACCOUNT IN IT WEBSITE. OK..
14) THEN AFTER THAT IN YOUR PROFILE , GO TO --> CHANGE SECRET QUESTION ANSWER OPTION & provide details as per your choice.

Without wasting any time you have to change the details of Contact number with your mobile number in the " CHANGE PROFILE CONTACT NO OPTION " OK..
15) AT LAST , AFTER THAT YOU CAN CHANGE THE PASSWORD GIVEN BY IT DEPT IN YOUR WEBSITE , AND USE PASSWORD AS PER YOUR CHOICE..
