Want delaration form for non-tds of transporter.

Sir, My friend has a business of giving rent of goods vehicle (tata ace) in a company. Now he has only 1 vehicle. He will filled first time ITR 4S in the A.Y 2017-18 as presumptive basis(Min 7500 /- pm per vehicle). He is receiving payment from company through RTGS, now he want to give a declaration to this company for not deduction of tds as per sec 194C(6) so how can he get this declaration form. Please give me information anyone or give me a proforma of this declaration form.
