select English Version

pls sir tell me the no. of sets & other documents to be submitted there

i have prepred 7 sets comprising of

1)vces form-1

2)calculation sheet (half yearly)

one copy of pan card & st-2 with covring page

what matter we have to written in the covring page pls specify
