Urgent Need help, Name Changed

Hello Friends. Myself Ashish Bhatt. I am a ca final course student of icai. I need help. My full Name is Ashish Bhatt. But in my School docucuments & with ICAI its just Ashish. A wanna change this to my real name with institute. please help me what is the process for this. Its urgent plz help me.. Is there any Legal Procedure for this or I need to just write a simple Application to Institute... Thanks in Advance
