Treatment of Ineligible ITC in 3B

Sir, we have ineligible ITC of rs. 2230 u/s 17(5) in Nov. month. But we confuse that should we show this ITC in both the ITC column i.e. 4(A) & 4(D) or should be shown in 4(D) ineligible ITC column only. I request u to please guide me what to do?
