Transfer after 1 yr of Articleship on medical grounds

Exactly what should be mentioned in the medical certificate? I mean that the duration of 3 months required should be mentioned on medical certificate or not? or should I take leave for 3 months first and then after taking leave, give the reason by producing medical certificate?
