Tds/form 16a from icici

Hi, I have been trying to get my form 16A from ICICI bank for last one month.

They are now saying that TDS certificate has not been generated only for UP customers. My form 26AS (TRACES) shows the ICICI TDS as 0, where as bank has given me a statement that Approx 41K was the interest for last financial and 4100 was the total TDS deducted in Q4.

I need the Form 16A/TDS details from ICICI  to file the tax as I am going out of country next week for a month and would like to complete filing my taxes ASAP.

Is there anyone else from UP who have not received their TDS/Form 16A from ICICI bank.

This is frustrating, Please help.
