already deposit in bank as salary and upload this to nsdl website and given form 16.

If you deposited tax through bank , then write an application to bank to change the sec.
If bank does your work then no need to inform AO. But if bank refuses(although it will not refuse) then write an application to Ao for change of sec. The application will be as if a normal application, ie no specified format is there.
