Preventive health checkup for self, spouse and dependent children upto an amount of Rs.5000/- is allowed under section 80D. Please clarify whether tax payer is required to have a medical insurance or without that also is he eligible to claim 80D.

No, it has only been mentioned that deduction is available upto rs 5000 for preventive heath checkup. While the amount is covered in the bracket of rs 25k, having an insurance is not necessary as per langauge of law.
