1) Designation & Address of CIT (TDS) - Click here for the link

2) Receipt Number: Receipt number to be quoted in Form 16/16A will be generated by TIN. Receipt number will be of 8 alphabets and will be applicable only for statements pertaining to FY 2010-11 and onwards. When you submit the TDS return, look for the receipt number on the top right hand side of the acknowledgement receipt and quote that 8 alphabets in the TDS certificate.

Basically its the Tax-deduction Account Number (TAN) of the deductor, Permanent Account Number (PAN) of the deductee, and Provisional Receipt number of TDS return (Mentioned in the acknowledgement as Token No.) filed by the deductor will form the unique identification for allowing tax credit claimed by the taxpayer in his income-tax return.
