I am filing Service Tax Returns for Q1 - can anyone help me to know how to add column in source document in challan service sheet, 4B.

I am trying to put source document details for interest paid, Interest is paid for 3 months, April, May & June.

after entering entry for April, I am trying to add new column for May and June-12, but as I click on add document button - I get the messege - EERORR PLEASE CONTACT ADMINISTRATOR.

Please help someone.

your STR no_premises code_ST3_MMYYYY.

eg: xxxxx_xxxx_ST3_042012.

After reading your reply I tried to add column but was unable to add. But as you said that you added the column just now, so I thought there must be some mistake in form. I downloaded the new form and filled again, this time column added properly and I filed my returns successfully. Thank you for your help.
