There is not a fixed format you can arrange the required information in any format. As under:-

no of shares
certificate no.
conclusive nos
nomination
Transfer(Purchase)
Transfer (Sale)

Well, there is no seperate format for private or public. you can either maintain in hard copy or soft as Register of members.
