Just an application on plain paper stating the reason for surrender is required. As an attachment, original certificate of registration is required to be enclosed. All the copies of returns filed earlier shall be taken along for verification.

Thanks Satish G, if Original Certificate is not with us so we can attached the status of Certificate (rom the Service Tax Web site.)

why there is no certificate of regn?? is it misplaced or you have not collected at all?? Certificate is required to surrender. If it is misplaced, sometimes the officer in the department ask for the FIR.

Hey jyoti, that means you have not collected it from the department. So just mention the fact in the application that you have not collected the Certificate. Other procedure is same as mentioned hereinbefore.
