Service tax refund

Hi,

 

1) In which case service tax refund is claimed on monthly basis and in which case it is claimed on quarterly basis?

 

2) Within how many days / months from the end of the above period(monthly or quarterly as the case may be) refund is to be claimed?

 

3) Which forms are to be used in above 2 cases for refund?
