Dear Sir,
It should contain follows.
1. Total Basic Salary
2. Total Allowances
3. Tax on Employment deducted
4.Othe item as may be appropriate

Salary certificate can be issued only if no tax was deducted on salary or salary does not cross the basic exemption otherwise issue form 16. It is a very simple form is no tax has been deducted. If the conditions above are fulfilled then the above format would do like name of the employee, total salary paid during the year Rs. ____, including allowances Rs. ____ less Profession tax deducted Rs. _____.
