Visit nearest regional office/Chapter office of ICAI along with 500 demand draft.You can get re-evaluation form from ICAI branch office and they will the DD to be drawn on.
Else there is online procedure as well
Online Procedure for Re-validation of Registration to ICAI Course
Step-1: Visit official website of ICAI Self-service portal on eservices.icai.org. Login to your account or create new one if you are visiting for the first time.
Step-2: You will see the text link to click here to access the students SSP portal.

Step-3: Click on Student Cycle as shown in the screenshot.

Step-4: Navigate Other forms from left sidebar Student Module. Thereafter click on apply for revalidation form as displays on the screenshot herein.

Step-5: Now fill up the revalidation form very carefully. Generate OTP and validate the same.

Finally, preview your revalidation form and click on submit button. After successful payment, your registration shall be re-validated to the respective term of 3 years or 4 years or 5 years respectively.
