Can I have a proforma Invoice of a Lease Rental to issue to my Tenant ?

Sir,
There is no prescribed format i.e
Name, Address, Date, Service-Tax No. if having, Gross Amt. Service Tax Amt. Ed. cess, S & H Ed. Cess, some naration which period, Ath. Sign., and some instructions which are mention on the invoice. It is not mandatory.
or search the Google. If you having the more observation in this regard plz forward the same.
