i am a commerce grduate student....& registered with CPT in July 2012

now my query is which option i have to fill in 1st point & 2nd Point :-

    b) Other Graduates
    c) Provisional Registration
    d) Conversion Scheme for existing CA Student



2) Conversion from    ( a) CPT (b) Intermediate (c) PE II (d) PCC (e) IPCC

   Registration No. (if registered Earlier) ...........Registration Year...........
