Could anybody please explain in detail about the G section fields(1-9) of Schedule FA, of ITR4 form, with examples?

What should be the (8)Shedule where offered and (9)Item number of schedule if the income is under 44AD? Is it Shedule BP/4 or Schedule BP/34(i)?
