1) Marriage certificate
2) School leaving Certificate
3) passport
(i) Certificate of identity in Original signed by a Member of Parliament or Member of Legislative Assembly or Municipal Councilor or a Gazetted officer, as the case may be (in prescribed format) ; or
(ii) l. Affidavit sworn before a magistrate stating the date of birth. - only for proof of date of Birth - but dont know for fther's name. Most probably will do.
