Why do you want a new card without disclosing about old card? Just fill up Form 49AB and give all the relevant details as were there on old PAN card. You will be issued a new PAN card with the same PAN number.

Why because he don't hv old card details, so he don't want to disclose.
does he apply without disclosing old card no in 49AB?

This link contains the required form. Non disclosure of PAN and getting a new PAN will create lots of problems for your client in future. Its not allowed to have multiple PAN. You just give old PAN number in the form. And all the actual details. If these informations get mismatched at IT Office, then you will need to give all the proofs of actual informations under the signature of a Gazetted officer. Just send these information with the form. You will get the PAN card with old number itself. I will not suggest you to go for a New PAN without disclosing old PAN.
