I have to apply for PAN for a government hospital.i want to know whether it falls under local authority/AJP/BOI/AOP

What documents are required for this like we provide birth certificate/adhaar for individual.

PAN card is not necessary for Govt Hospital. If you want to apply for GST apply with TAN. If you want to file TDS return use PANNOTREQD in the place of PAN of the deductor.
