Should I mention my address in client's application form for PAN Card?

No, The Address should be in Clients Name for PAN Card Application

A Chartered Accountants address can be mentioned as communication address in the application only by Foreigners having address abroad. That too they have to give a Power of Attorney signed before our Indian Consulate in favour of the Chartered Accountant. Details available at tin.nsdl.com
