For presumptive taxation, u have to file in ITR-4S. Rule 12(1)(ca) states the following :
[(ca) | in the case of a person being an individual or a Hindu undivided family 42a[or a firm, other than a limited liability partnership firm,] deriving business income and such income is computed in accordance with special provisions referred to in section 44AD and section 44AE of the Act for computation of business income, be in Form SUGAM (ITR-4S) and be verified in the manner indicated therein: |

Then use ITR-4, giving gross receipts and profits alone in profit & loss account, in the place provided for those who does not maintain any books. In the Schedule - BP, specify that the profit is deemed to be under 44AD
