FOR ISSUE OF NEW PAN CARD AGAINST LOST ONE
WE HAVE TO FILL FORM FOR NEW PAN CARD/ CORRECTION IN OLD ONE
BUT IN THIS FORM , THERE IS NO COLUMN FOR LOST PAN CARD.
THEN WHAT WE DO

Not a problem. You can attach a copy of GD / FIR lodged with police station. Some TIN FCs do not require that too. Mention the PAN in form and enclose the address and identity proof docs. You have to use the "correction" form.

YOU NEED ONLY NEW PAN CARD COPY, SO YOU HAVE NO NEED TO SELECT ANY OPTION IN PAN CHANGE FORM
JUST SUBMIT THE FORM WITH RELAVENT ID AND ADDRESS PROFF AND COPY OF ORIGINAL PAN CARD.
