Please tell me on 108 form firms name will be given ????

For exam form whose seal will be affixed.

This same thing happened to me. The important thing is that you are registered the Principal (individual) and not the firm.
This is what I did:
In the first page where the principal will write his firm name he will write his new firm name.
In the report of practical training i wrote both firm's names first the new one and then the old one. Same with the firms registration number. Naturally the blocks to fill were less but i wrote them both any way (in block letters).
Today I am a member of ICAI, so goodluck.
