Late submission of form 112

hii. i had completed my b.com in 2011 & now applied for m.com ..and there is a problm of sending form 112 now..if i send 112 with in 60 days of admission in m.com..then what fees should be payable..and b.com marksheets as evidence for completion of b.com..and is there any other requirement of paper work with icai..plz help me out..thanx in advance
