what is the form of GST return in case of job work

When principal send goods to Job work through delivery Challan , The details of the Goods dispatch to a job worker or received form job worker or sent form one job worker to another job worker during a quarter shall be furnish in form GST-ITC04 on or before 25th of of the month succeeding the said quarter.
