Itr4 or 4s or i???

Dear  Friend

1) one of my friend  provided technical service for 9 months to co. Co also deducted the tds under 194j, and after that he left giving service and  joined other co  and did job where he got salary.

so please tell which form is used for it ,,, and also can he use section 44ad for technical service or  not ????

2) which form is used for Lic agents?????
