LAST DATE FOR ITC 04 FOR JULY 17 TILL 30TH SEP 18 HAS BEEN POSTPONED TILL 31ST DEC 18. ITC 04 HAS TO BE FILLED QUATERLY.
SO NOW YOU CAN EASILY UPLOAD THE RETURN FOR :
JULY 17 TO SEP 17
OCT 17 TO DEC 17
JAN 18 TO MAR 18
APR 18 TO JUNE 18
JULY 18 TO SEP 18

Yes. The last date has extended as mentioned above. But this form is half-baked and not fully ready causing needless troubles to tax-payers. Request some experts to take the matters with relevant authorities.
Some common scenarios where this form does not work (because a single original challan number cannot be given):
Scenario 1
Principal sends input goods A to jobworker on issue challan 1.
Principal sends input goods B to jobworker on issue challan 2.
Principal receives single finished goods C made of A + B from Jobworker on receive challan 1.
Scenario 2
Principal sends input goods A to jobworker on issue challan 1.
Principal sends input goods B directly from supplier to jobworker on issue challan 2
Principal receives single finished goods C made of A+B from Jobworker on receive challan 1.
Scenario 3
Principal sends input goods A to jobworker on issue challan 1.
Principal sends input goods B to jobworker on issue challan 2
Principal sends input goods C to jobworker on issue challan 3
Principal receives finished goods D from Jobworker which is made out of part of input goods A and entire input goods B on receive challan 1
Principal receives finished goods E from Jobworker which is made out of part of input goods A and entire input goods C on receive challan 2

These are common scenarios in most businesses. In all these cases, it is not possible to give a single original challan number.
Hence someone should takeup this issue with relevant authorities and perhaps suspend this for 2017-18 and current year, update the form to be able to handle scenariios mentioned above and perhaps many more and implement it from 2019-2020.
