Can anybady tell me wheather i will have to send any thing like completion of article ship certificate with Print out of exam form , bcoz this time in instruction nothing has been written regarding the same

Yes, you have to attach an true copy of Articleship completion Letter. It is mentioned of the Service Certificate (3rd page) where you have to sign an declaration of completion of articleship & the date of completion.
