Ipcc online form doubt -urgent plz help

Dear Friends,

When I filled ipce exam form for previous attempt i.e may 2014, there was a field in the online form where a candidate had to enter the details of the ITT CERTIFICATE.

But when I filled for this attempt i.e November 2014, I could not find any such field requiring me to enter ITT certificate details.

Has ICAI modified the form? please share your the information.
