Where to enter Input services tax amount (input on telephone & mobile bills ) in ST-3 return form of July - Sep 2013?

if you see the return there is a seperate sheet for CENVAT Credit. we need not give the input details service wise. We need to mention the consolidated amount of service tax, Education Cess and Higher cess
