Income tax refund ay 2012-13

hello friends,

my relative filed manual itr 4 for the ay 2012-13 at income tax office, amritsar. my relative is basically from sundernagar (mandi, himachal pradesh). afeter filing the return when we enquired abt the inc tax refund, we came to know that the return has been transferred to mandi, himachal pradesh. i dont know why this happened wen we had mentioned amritsar's address in the itr4. now its may 2014 and my relative hasnt received his it refund yet and the period to file belated or revised return has also expired. the acknowledgement number of the itr4 filed is also missing. pls guide me what steps shud be taken to get it refund for the ay 2012-13. shud we write a letter to ito mandi regarding this ?... if yes then pls explain full procedure for the same.. whom shud i address the letter, do i need to fill ward no, range etc... where can i get to know abt all these ?
