Whether LIC Premium Receipt is a valid ID/Address Proof for applying PAN or can we give an Affidavit attested by a notary for ID/Address Proof?

U HAVE TO GIVE VOTER ID, DRIVING LICENCE, RATION CARD OR PASSPORT PROOF....

if you are not having anything other than just lic receipt you can use

Affidavit attested by a notary

or if you are having bank account

copy of front page of your passbook containing your address (with stamped by bank)

if u want to submit an add or id proof for obtaining of pan u must give him the following documents : -

for address proof:

rent Recpts

voter i card/ DL/ Electricity bill/mtnl telephone bill/passport copy

written by any G. OFFICER for ur add.

FOR ID PROOF:-

V. I. Card/dl/passport copy/10th passed certificate/ written by any G. Officer about u.
