You can take a letter on plain white paper as follows:
To
A/c Dept.
Name of Firm.
This is to confirm that I Mr.____________ residing at _____________has given Add:_____________ on rent to Mr.(Name of employee)_ from perio___________ to ___________.
Rent amount payable for the period is Rs.____________
I further declare that I am not having PAN no. under Income Tax act.
This leeter is issued on request of Mr. __________

Pl note from declaration it should be clear that it is issued to clear that he is landlord & he has given particular property on rent to you.
