There is no specific format for internal audit report it depends on the scope & objective of your audit.

U have to report according to requirements of your client.

Present the report in PPT format :-

Each Slide should contain the below details

1. Issues or audit observations as heading.

2. Write observations or explain the issue

3.Risk & Impact.

4.Root Cause.

5.Recommendation.

6.Ask For Replies from the management and type the Comments.
