How to fill challan for demand notice-intimation us 143 (1)

I tried to fill the challan ITNS 280 for the assessment year 2013-2014, Financial Year 2012-2013 for the demand notice of 640 Online through income tax website.

But when I filled and when the link re-directed me to bank website I got few fields as below, and I am not sure what has to be entered in each field

  1. Income tax
  2. Surcharge
  3. Education Cess
  4. Interest
  5. Penalty
  6. Others

Anybody who has filled, please guide me.
