1st condition you have adopt 12%gst rate service that you allowed input tax credit
2nd condition you have adopt 5%gst rate service than you have not allowed input tax credit

No you cannot adopt both the condition ,... any one can choose in the begining of Fy & has to carry the same till fy end . so first of all correct it in books of account then go for return filing
