How to correct mistakes in intermediate form

I made some mistakes like in first option - Route for joining Intermidate  if filled CPT in place of foundation and in option no 12  (date, & month and year of passsing) are filled incorrectly by mistake while filling intermediate form, so can i cange it now. If yes then how ?
